How to Create a MyProviderLink Account From a Paper Bill

In order to create your account, you will need the account number and Form ID from your bill. These, along with the link to
www.MyProviderLink.com can be easily found at the top right of your bill.
Sign Up
Complete the Sign Up page information, including your account number, Form ID, email address, and create a password.
Password must be 6 - 15 characters
You can also set your Bill Delivery Preferences. Want to receive your bill via mail, email and text? Simply select "Yes" for all three.
