How to Create a MyProviderLink Account From a Paper Bill

Overview

You can create a MyProviderLink.com account either from a paper bill you receive in the mail or from an eBill Notice that you receive via text or email.  This article covers how to create the account from the traditional paper bill.

Account Number and Form ID

In order to create your account, you will need the account number and Form ID from your bill.  These, along with the link to www.MyProviderLink.com can be easily found at the top right of your bill.

Sign Up

Go to www.MyProviderLink.com and click the Sign Up button.

Complete the Sign Up page information, including your account number, Form ID, email address, and create a password.

Password must be 6 - 15 characters

You can also set your Bill Delivery Preferences.  Want to receive your bill via mail, email and text? Simply select "Yes" for all three.
